MGT Policies

As the parent or legal guardian of the listed gymnast(s), I hereby consent to the named person(s) participating in the programs offered by MG Tumbling.  I recognize that potentially severe injuries including virus, sprains, broken bones, paralysis or death can occur in any activity involving height or motion, including power tumbling.  I understand and accept this risk.  Additionally, I have communicated the risk to my child/participant.

Further, I realize that my child will be performing and training on all events, equipment and devices including the Double-mini and tumbling trampolines. I realize that while payment of tuition and registration fees constitutes a part of the consideration due to MG Tumbling for allowing my child to use the facilities and equipment at Northern Star Bounders/MG Tumbling, an additional part of the consideration is this signed release form.  Therefore, in consideration for allowing my child to use the facilities at Northern Star Bounders/MG Tumbling, I hereby release Northern Star Bounders/MG Tumbling, it’s owners, officers employees and coaches from all liability for any and all damage, illness and injuries suffered by my child while under the instruction, supervision or control of MG Tumbling, it’s owners, officers, employees, and coaches.

As the parent or legal guardian of the aforementioned person(s), I hereby agree to individually protect for the possible future medical expenses which may be incurred by my child as a result of any injury sustained while training at, for or under the direction of Northern Star Bounders/MG Tumbling.  In addition, I confirm that my child has been examined by a physician who has cleared them for unrestricted participation in these activities.  This acknowledgement of risk and wavier of liability, having been read thoroughly and understood completely, is voluntarily signed as to its content and intent.

I understand that the nature of the sport poses risk.  In the event of serious injury or illness, when the parent cannot be reached immediately, I hereby authorize the coaches of MG Tumbling to obtain emergency medical care from physicians, paramedics, or other authorized emergency agents for my child and agree to pay for said care.

Payments for the upcoming month will be posted on the 15th of each month. For example, March’s payments will be available to be viewed no later than February 15th. They can be paid at any time, but all payments not fulfilled by the 1st of each month due will be automatically withdrawn from the credit card on the account file. If your payment is unsuccessful or declined by our payment processing software, please inform Delana. Failure to notify her will result in a $10 monthly fee due to the declined transaction.

Registration Fee

Registration is due for all Students in the program on September 1st. New students will pay in the month of enrollment and payment will be pro-Rated based on the month enrolled.

Team Registration: $125

Recreational Registration: $30 (Jan-Aug:$20, June-aug: $8)

Tuition Rates

Tuition is based on the calendar year and is not Pro-Rated based on the number of lessons in the month for existing students. Recreational students will receive a Pro-Rated rate if the first month they enroll has already had a lesson taught before enrollment.

Team Tuition: $115

Recreational Tuition: Once weekly, $55 Twice weekly, $105

 * Cancellation policy is 2 weeks notice. Athletes will be charged for the remainder of the 2 week frame. All athlete withdrawals must be done directly through Delana Nelson (801)791-4163 TEXT or CALL! Failure to do so will result in continued registration and charges for classes, with NO REFUNDS.

If I do not pay BEFORE the 1st day of the month, I give MG Tumbling permission to charge my account for my monthly tuition as well as any applicable Registration Fees, Competition Fees and/or Uniform costs that are due.  I understand that all times I must keep Active credit card on my account file.  I understand that I will be charged $10 per month for declined credit cards.  I understand that failure to complete payments will result in my athletes not being allowed to participate in class.  I understand that notification of cancellation is 2 weeks prior to cancellation date and I will be charged all applicable fees for that 2 weeks period.

The staff of MG Tumbling loves the opportunity to work with your children and we enjoy watching them progress in the sport.  From time to time MG Tumbling features our athletes in publications, social media and/ or video productions.  These are used to advertising and gym information.  I give MG tumbling permission to use my child’s photo or image on material or posts done ONLY BY MG Tumbling.
